Strengthening the Workforce

Caring Gene® Career Pathways Training (CPT) Program participants commit to a three-year service agreement with an eligible New York State healthcare, behavioral health, or social care provider serving Medicaid and uninsured populations or participating in a Social Care Network. As participants enter the workforce through spring 2027, they will help address critical workforce shortages and strengthen care in communities across New York State.

Healthcare Provider Eligibility Definition

A healthcare provider is an organization that delivers direct physical health or behavioral health services to patients, clients, or community members. For purposes of The Caring Gene Career Pathways Training (CPT) Program, healthcare providers may be eligible to become an Approved Employer if they employ individuals in qualifying healthcare professions and meet program requirements.

Physical Health Providers

Physical health providers offer medical, nursing, rehabilitative, diagnostic, or other healthcare services that support an individual’s physical well-being. Examples include:

• Hospitals and health systems
• Federally Qualified Health Centers (FQHCs)
• Primary care and specialty medical practices
• Skilled nursing facilities and nursing homes
• Home health agencies
• Hospices
• Rehabilitation centers
• Diagnostic and treatment centers
• Public health organizations

Behavioral Health Providers

Behavioral health providers deliver services that support mental health, substance use treatment, and emotional well-being. Examples include:

• Mental health clinics
• Substance use disorder treatment programs
• Community behavioral health organizations
• Certified Community Behavioral Health Clinics (CCBHCs)
• Outpatient counseling centers
• Residential treatment programs
• Crisis intervention services
• Integrated behavioral health programs

Sample Attestation Form

To become an approved service commitment site, you must submit an attestation form. Please Note: You cannot submit the attestation form PDF; it is for viewing purposes only. Please contact Alyse Buccina to discuss the attestation process in more detail. If possible, please also include a point of contact in your email to Alyse.

Provider Responsibilities

• Complete the interest form, located above.
  • Please note that New York State requires a separate attestation for each site location, even if all sites have the same MMIS/Medicaid percentage data.
• The Provider Partnership Specialist will reach out to schedule a brief informational meeting.
• Identify an internal point of contact for program management and communications.
• Sign the attestation.
• The Provider Partnership Specialist will submit the attestation to the New York State Department of Health.
• Upon approval, the designated contact from your organization will receive instructions to post on a private job board.
• Providers are eligible for backfill payments to compensate for coverage required to fill a student shift for up to two shifts a week.
