Strengthening the Workforce

As part of their enrollment in the Caring Gene® Career Pathways Training (CPT) Program, participants commit to a three-year service agreement with a healthcare, behavioral health, or social care provider in New York State that serves a patient population of at least 30% Medicaid/uninsured (including dual eligible), OR is a contracted provider with a social care network.

CPT Program participants will enter the workforce now through spring 2027, addressing shortages, enhancing patient care, and supporting communities in need throughout New York State.

Become a Service Commitment Site

To become an approved service commitment site, you must submit an attestation form. Please Note: You cannot submit the attestation form PDF; it is for viewing purposes only. Please contact Alyse Buccina to discuss the attestation process in more detail. If possible, please also include a point of contact in your email to Alyse.

Provider Responsibilities

• Complete the interest form, located below.
  • Please note that New York State requires a separate attestation for each site location, even if all sites have the same MMIS/Medicaid percentage data.
• The Provider Partnership Specialist will reach out to schedule a brief informational meeting.
• Identify an internal point of contact for program management and communications.
• Sign the attestation.
• The Provider Partnership Specialist will submit the attestation to the New York State Department of Health.
• Upon approval, the designated contact from your organization will receive instructions to post on a private job board.
• Providers are eligible for backfill payments to compensate for coverage required to fill a student shift for up to two shifts a week.
