Patient Care Managers
Patient Care Managers (PCMs) are healthcare professionals responsible for overseeing the delivery of high-quality care to patients within a medical facility, such as a hospital, clinic, or long-term care facility. This role involves managing healthcare staff, coordinating patient care plans, and ensuring compliance with healthcare standards and regulations.
Overview
Patient Care Managers (PCMs) identify high risk patients in ambulatory settings, conduct assessments of clients, prioritize areas of concern to identify potential areas for intervention, assess patient priorities and share information with care teams. Responsible for establishing positive relationships with patients, effectively communicating a patients’ needs, and discussing the available/appropriate options and resources. PCMs may follow up on recently discharged patients, develop action steps to meet care plan goals, identify barriers to care plan implementation, communicate the goals of a care plan, develop a timeline for implementation, document care plans, work with patients to establish agreed upon goals, initiate services and interventions before acute care episodes occur, and evaluate client satisfaction with services. They may provide clinical support, including supporting Primary Care Physicians (PCPs) to achieve integrated care, participate in peer review and/or clinical supervision, complete reports and data analysis to determine the effectiveness of interventions, and monitor service delivery and interventions.
